The front-desk work worth automating.
Most clinics lose patients in the gaps between systems rather than in the chair. A website appointment request sits in an inbox until someone checks it. A missed call during a busy hour is never returned. A patient forgets an appointment because the reminder depends on someone remembering to send it. A no-show is not offered another slot. Each of these is a small, rule-based job that n8n can run reliably, while the front desk keeps control of anything that needs judgement.
- Website and WhatsApp appointment requests into one queue
- Appointment reminders and confirmations
- Missed-call text-back during busy hours
- No-show and cancellation follow-up with a rebooking link
- Post-visit review requests after staff confirm the visit
- Daily summary for the front desk
A worked example: a three-chair dental clinic.
This example is illustrative, not a client case. A three-chair dental clinic takes requests by phone, a website form and WhatsApp. In the n8n version, every website form and WhatsApp request lands in one queue with the patient's name, phone number, preferred time and reason for the visit. The workflow checks whether the phone number already belongs to a patient in the practice-management system or CRM, creates a request record, and sends the patient an acknowledgement. The receptionist sees the request with a suggested slot and confirms it; only then does the workflow send the confirmation and schedule reminders 48 hours and 3 hours before the appointment. If the patient does not attend, the workflow waits until the end of the day, then sends a message offering to rebook and adds a task for the receptionist. After a completed visit, and only once staff have marked it complete, the patient receives one review request. Clinical notes never pass through the workflow; it carries only what is needed to book and remind.
- Inputs: website form, WhatsApp Business messages, missed-call log
- Checks: existing patient match, requested time within opening hours
- Staff step: receptionist confirms the slot before any confirmation is sent
- Outputs: confirmation, two reminders, no-show rebooking task, one review request
- Excluded: clinical notes, diagnoses and treatment details
Patient data and messaging rules.
Clinic workflows handle personal and often health-related data, so the design starts with what the workflow is allowed to see. A self-hosted n8n instance keeps workflow data on infrastructure you control, which helps with data-location requirements, but it does not by itself make a workflow compliant. Depending on where the clinic operates, rules such as India's Digital Personal Data Protection Act, the UAE data protection law or HIPAA in the United States may apply; the clinic and its advisers decide which apply, and the workflow documents what data it uses, where it is stored and who can access it. WhatsApp reminders sent more than 24 hours after a patient's last message must use pre-approved WhatsApp Business message templates, so templates are prepared and approved before launch. Patient consent for reminders and review requests is recorded and respected.
How the project runs.
A clinic project follows the same scoped steps as every Marsen n8n engagement, with one extra step for message templates and consent.
- 1. Front-desk review: how requests arrive today, opening hours, systems and staff roles
- 2. Scope: which workflows, data fields, staff approvals and message templates
- 3. Templates and consent: WhatsApp templates submitted for approval, consent wording agreed
- 4. Build and test: synthetic patients only; duplicate, out-of-hours and no-show cases
- 5. Pilot: one workflow live for two weeks, reviewed with the front desk
- 6. Handover or support: runbook, staff guide and optional monitoring
Which systems it connects to.
n8n connects to most systems that have an API or webhooks: practice-management or booking software, Google Calendar, a CRM such as HubSpot or Zoho, WhatsApp Business Platform, email, SMS providers and Google Sheets for simple reporting. Where a practice-management system has no API, the workflow can still manage requests, reminders and follow-ups around it while staff enter the confirmed booking. We check your actual systems on the first call before quoting.
Pricing.
A first clinic workflow, such as appointment requests with reminders, is a fixed $1,799 (USD, excluding tax). Monitoring and support for live clinic workflows is $249 per month. Additional workflows are quoted once the first workflow is live. Third-party costs such as WhatsApp conversation fees, SMS and hosting are listed separately in the scope. In India, the missed-call, WhatsApp booking and reminder setup starts at ₹39,999 plus ₹4,999 a month (excluding GST); see the AI receptionist for clinics page for what it includes.

Talk it throughCan n8n send WhatsApp appointment reminders?
Yes, through the WhatsApp Business Platform. Reminders sent more than 24 hours after the patient's last message need pre-approved message templates, which we prepare and submit before launch.
Does the workflow see patient medical records?
No. The workflow is designed to carry only what is needed to book, remind and follow up: name, contact details, appointment time and a short reason. Clinical notes stay in your clinical system.
Is self-hosted n8n HIPAA or DPDP compliant?
No software is compliant on its own. Self-hosting keeps data on infrastructure you control, which helps, but compliance depends on your policies, agreements, access controls and how the workflow is designed. The clinic and its advisers decide which rules apply; we document the data flows to support that review.
Will an AI reply to patients automatically?
Only where you approve it. By default, confirmations and anything beyond a simple acknowledgement wait for a staff member to confirm. AI can draft a reply or summarise a request for the receptionist.
Do you work with clinics in Dubai or the UK?
Yes. Marsen works remotely. Messaging providers, data-location requirements and time zones are agreed in the scope.
How much does it cost for a clinic in India?
In India the missed-call, WhatsApp booking and reminder setup starts at ₹39,999 plus ₹4,999 a month, excluding GST. WhatsApp message charges and phone-line fees are paid directly to those providers.
How quickly can a clinic start?
After a short front-desk review and scope, the first workflow is built and tested with synthetic patients, then piloted for two weeks. WhatsApp template approval can add a few days.
